For patients with more complex dental or medical histories, a follow-up appointment may be required to complete treatment.
To ensure a smooth and efficient visit, please be prepared to provide the following at your initial dental consultation:
- Your referral slip
- A list of any medications you are currently taking
- Dental insurance information submitted prior to your appointment—this helps us verify coverage and streamline claim processing
- Completion of the Patient Registration Forms on our website prior to your visit. You will receive an email with login details.

- In order to provide care as safely and effectively as possible, a complete medical history is essential
